THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY. HH understands the importance of the privacy and security of your health information. HH utilizes the medical information of its clients in order to provide the services outlined within our Terms of Service agreement. At all times HH is required by law to maintain the privacy of protected health information, to provide individuals with notice of our legal duties and privacy practices with respect to protected health information, and to notify affected individuals following a breach of unsecured protected health information. How HH uses or discloses your health information: In order to provide services to you the client, HH is required to both collect and use your medical information via authorized transfers from your electronic health record, medical health questionnaires that you complete and/or medical test results that you transfer to the HH system. HH only discloses your protected health information to healthcare providers that you have explicitly authorized and consented to within your online HH profile or as required by law enforcement where there is a legal obligation to do so. The HH system uses components of all contained health information which does not under any circumstance identify you for statistical aggregate reporting purposes. One of the services of HH is the ability to receive notifications. These communications may be from government or private health organizations, health information providers, suppliers of medical devices, medication or other health-related products and services. You will only receive such communications if you specifically request the information and you may opt out of receiving such notifications at any time. If you opt out for such communications, the content will be exclusively delivered via notifications within your online HH profile. How HH shall not use or disclose your health information: HH does not under any circumstance use information that could identify you for profit. Your protected health information is not collected, used or disclosed in any manner of which you have not provided consent. Your rights as a client of HH: As a client of HH, you have rights to request a copy of your information as it exists within the HH system as well as request any corrections to your information. Should you make a request of this nature, HH is committed to responding within a 30-day period and furthermore we may charge a reasonable cost-based fee related to your request for a copy of your health information. In the unlikely scenario that HH denies a request to correct any of your health information you will receive a confirmation on this decision within 60 days in written format. If at any time you choose another individual with medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information as permitted by law. In this scenario, HH will take steps to ensure this person can produce proof of such legal status prior to fulfilling any requests to access your personal information. If at any time you as a client of HH wish to discontinue use of the HH system, please contact support@harrisonhealthcare.ca and we will ensure the removal of your account within a 30-day period. After removal of your account has occurred, your information may be retained by the HH system for a period of time as required by medical record retention laws however it will never be accessible by individuals. After the required period of time has passed, HH will securely dispose of your personal information. HH is obligated to provide you with information on our confidentiality practices should you request it or should you have any questions or concerns. Please make such requests to the e-mail address provided in the header of this notice. Protection of your information: HH places a high importance on protecting your information from unauthorized use or disclosure. All connections to the HH system are protected using SSL encryption and the health information that is retained by our system is encrypted to ensure confidentiality. The HH website is certified and regularly monitored by McAfee SECURE and WebsiteServices.org services. HH regularly undergoes penetration testing, vulnerability, risk and privacy assessments by qualified third party firms in order to ensure the privacy and security of its system. Changes to this Notice of Privacy Practices: When HH changes the terms of this notice which affects the way we treat your information, we have an obligation to inform you via e-mail and also update the text of this notice on the HH web site. Complaints: If you wish to issue a privacy complaint to HH, you can issue such complaints to the e-mail address provided in the header of this notice. You may also file a complaint to the U.S. Department of Health and Human Services Office for Civil Rights, please see www.hhs.gov/ocr/privacy/hipaa/complaints/ for further information. If you are outside of the United States, you may issue a privacy complaint with your relevant data protection officer or privacy commissioner. At no time will HH retaliate against you for filing a complaint.
